We focus on seamless transitions, safe discharges, and collaborative care planning--all while keeping patient well-being and healthcare excellence at the forefront.As a Care Manager, you'll play a vital role in guiding patients through their hospital journey--from admission to discharge. You'll work closely with physicians, nurses, and other healthcare professionals to ensure every patient receives the right care at the right time. You'll also have the opportunity to support patients with complex or high-risk psychosocial needs, making a meaningful difference in their lives.What You'll Do:Lead and coordinate discharge planning from day oneCollaborate with a multidisciplinary team to ensure smooth patient transitionsSupport patients with complex needs, offering compassionate and strategic careDrive performance through best practices and data-informed decisionsUphold Mercy's mission, values, and Service Standards in every interactionIf you're passionate about improving patient outcomes, thrive in a collaborative environment, and want to be part of a team that truly makes a difference--we'd love to meet you.Qualifications:EducationBachelor's Degree of Science in Nursing LicensureCurrent License in the state of employment, required Experience2-3 years acute care hospital setting, preferredCare Management or Utilization Management experience, preferred Certification/RegistrationBLS (CPR) at hire date, Minimum Required, or within 90 days of hireCertification in Case Management, Preferred Other Skills & KnowledgeStrong verbal and written communicationStrong assessment, time management, and critical thinking skillsInteracts effectively with a variety of people and situations at all levels of the organizationThe assessment, care, and treatment will be consistent with the specific age-related needs of the patient.
